If you or someone you know could use some help this Thanksgiving, please fill out the form below with the information of the family receiving the basket by November 15th. We will be delivering Thanksgiving baskets on Saturday, November 22nd from 10 am to 12 pm.

Awaken Church, sign up below to help assemble & deliver Thanksgiving baskets on Saturday, November 22nd. We appreciate everyone who lends a hand in making this possible. We will meet at the church at 9:30a to assemble the baskets and then hand-deliver them between 10a-12p.
